Abstract
CAPD patients present special problems with respect to vitamin D metabolism. Because of substantial losses of vitamin D metabolites in the peritoneal dialysate, they require 2-3 times the oral replacement doses used in hemodialysis patients. Administration of 1,25(OH)2D3 or 1-alpha-OH-D3 should be sufficient to maintain normal levels of 1,25(OH)2D3, and to stimulate the renal production of 24,25(OH)2D3. However, anephric patients require, in addition, a source of 25OH-D3, in order to enhance extrarenal production of 24,25(OH)2D3.
