A 65 yr old man with hypophosphatemic osteomalacia is presented. The clinical features of this syndrome include bone pain, muscle weakness, pathological fractures and radiological evidence of osteomalacia. The main biochemical findings were normocalcemia, hypophosphatemia and hyperphosphaturia. Treatment with oral inorganic phosphate and 1 alpha hydroxy cholecalciferol resulted in normalization of plasma phosphate and marked clinical and radiological improvement
