Mapping the landscape of naturopathic doctors' increasing role in American health through vaccination

Strain on industrialized healthcare systems has increased the need for non-physician health professionals, including naturopathic doctors (NDs), a rapidly growing group trained for primary care with an emphasis on wellness and disease prevention. ND education includes foundational biomedical sciences but differs substantially from MD/DO training in clinical hours, residency requirements, and regulatory oversight. Licensure and scope of practice are not determined federally, and NDs currently lack federal recognition as classified physicians. Despite these constraints, the ND workforce doubled between 2000 and 2016, positioning the profession to exert increasing influence on public health. Through the case study of vaccination, the influence and insight of NDs can be analyzed. Immunization uptake is closely linked to public trust in health institutions and serves as a key indicator of alignment between individual-centered care and population-level public health goals. International research on naturopathy, while limited, suggests cautionary trends, including declining willingness among students to recommend full vaccination as training progresses and lower vaccination rates among populations who consult naturopathic or similar practitioners. While current evidence does not conclusively demonstrate that NDs promote anti-vaccination practices, these findings highlight a critical gap in empirical research and oversight, and raise questions regarding the consistency of care, patient autonomy, and collective health outcomes. As naturopathic medicine grows and seeks increased clinical autonomy, empirical research and regulatory frameworks will be essential. Such efforts can help balance respect for practitioner autonomy and patient choice with ethical obligations to uphold evidence-based public health standards.
