Medication errors: A common failure in physician-patient communication

Abstract

Errors may occur in the complex process of medication use, particularly in the prescribing and administration phases. This article briefly reviews common errors, clinical and regulatory recommendations made in recent years which have resulted in safer medication use and some of the new challenges that have emerged.
